Most forms for a new facility\, transfer\, renewal or temporary facility can be found here. 							\n						\n					\n											\n														\n								The Open Public Records Act (OPRA) is a State law to increase the public’s accessibility to view and/or obtain a copy of government records maintained by public agencies in the State. If you would like to submit an OPRA request\, you must go through the proper process. An OPRA records request is not considered complete unless it is submitted to the custodian who oversees the record. Visit the State of New Jersey’s OPRA website to submit a complete request.
